The Orange County Care Coordination Collaborative for Kids (OCC3 for Kids) is a partnership of more than 30 organizations working to improve systems of care for children with special health care needs. Funding to support this partnership is provided by Lucile Packard Foundation for Children’s Health along with in kind contributions of staff support from those who participate on OCC3 for Kids.

Transition of Care Toolkit

OCC3 for Kids is working to create seamless transition from services for youth with special health care needs to services for adults. We have developed a Transition Toolkit, based on the Got Transition model and tailored to Orange County, that includes components for health care providers, youth, parents and service providers. The Transition Toolkit, which can be customized, includes these elements:

OCC3 for Kids has compiled a list of resources related to transition age youth to help with everything from health care skills and health services to school, employment, independent living, and community resources.
